A lower abdominal midline incision was made and the abdominal wall opened in the usual fashion. Upon entering the abdominal cavity, no unusual peritoneal fluid was noted. The upper abdomen was explored and found to be within normal limits. There were noted bilateral large ovarian endometriomata. The anterior cul-de-sac was free of adhesions, but the lower part of the sigmoid colon was adhered to the posterior wall of the cul-de-sac. The sigmoid adhesion was taken off the posterior wall of the uterus by sharp and blunt dissection , down past the uterosacral ligaments, freeing the cul-de-sac area. The bilateral endometriomata were ruptured, freeing the ovaries from the lateral pelvic walls. The round ligaments were bilaterally clamped, cut, and ligated with #1 chromic sutures. The visceroperitoneum between the round ligaments was transversely incised and the bladder bluntly dissected off the lower uterine segment of the cervix.
