1. Open Office Hours.
2. Click Appointment List on the Office Hours Reports menu.
3. Select the option to print the report on the printer.

5. Enter 12112010 in both Dates boxes.
6. Enter 6 in both Providers boxes.
7. Click the OK button.
8. When the Print dialog box is displayed, click the OK button.
