According to the package insert, the recommended dose for this diagnosis for patients with normal renal function is 10 mg/kg q8h. The dose is adjusted as follows for patients with impaired renal function: for creatinine clearance over 50 mL/min, 100 percent of the recommended dose every 8 h; from 25 to 50 mL/min, 100 percent of the recommended dose every 12 h; from 10 to 25 mL/min, 100 percent of the recommended dose every 24 h; for 0 to 10 mL/min, 50 percent of the recommended dose every 24 h.
