Petit recently had a battery of tests to rule out cardiovascular problems after experiencing a rise in his serum cholesterol level and shortness of breath. His primary doctor planned to refer him to follow up with a cardiologist on his next visit. Mr. Petit never returned for his follow-up appointment with his primary provider (a general practitioner) at the community-based clinic in his neighborhood. After his second missed appointment, his primary doctor made a home visit. He questioned Mr. Petit about his missed appointments, reviewed the results of his diagnostic tests, and completed a quick examination. Mr. Petit, feeling tremendously improved since adding an extra glass of red wine to his diet every day, feels no need to be seen so often at the clinic. He calls the wine a protection against a heart attack and believes it is a waste of time to see a cardiologist as he has no pain.
