Mr. Schwartz is undergoing treatment for a blood clot in the leg. He has been taking Coumadin , an anticoagulant, for several days. He is also taking Diuril , a diuretic, to reduce the swelling in his leg. The doctor has confined Mr. Schwartz to bed. This is because any movement of the leg might cause the clot to break off and travel in the bloodstream, which could be dangerous.
At 0800 on October 5 you give Mr. Schwartz two 5-mg tablets of Coumadin to swallow with a glass of water. You chart it on the medication administration record. As you refill the water pitcher, Mr. Schwartz asks if you happen to have something for an upset stomach. He says his stomach hurts the way it did when he had an ulcer 20 years ago. Because the anticoagulant can cause internal bleeding (e.g., from an old ulcer), you have been alert for signs of this side effect. You notify your supervisor, who asks the doctor to order a test for internal bleeding. The test results are negative.
