Five middle managers, all approximately the same age, are holding a series of meetings aimed at reforming the company's disciplinary code.They know that they are all well-organized professionals, and they assume that they can mentally keep track of developments and decisions that arise in the meetings; there are therefore no agendas or minutes.Moreover, the most confident manager in the meeting has successfully convinced the others that this is all a waste of time, anyway, because HR will never agree to changes.The end result is that the existing disciplinary code has stayed in place, without even the minor changes the managers recommend, since they forgot to implement these changes.
